Travel insurance is sold as one product, but what it has to do for you depends on where you are going. For a US traveler the same policy faces a different job in each country, and four things do most of the work in deciding whether it pays.
Medical billing. Medicare and most US health plans do not follow you abroad, and hospitals in many countries expect payment or a deposit before treatment. Whether a policy pays the hospital directly or reimburses you later matters more in some places than others.
Evacuation distance. A ferry-linked island, a mountain valley or a trekking route can be hours from a hospital that can treat a serious injury. Evacuation and search-and-rescue wording is what covers that gap, and it is where policies differ most.
Entry rules. A few destinations ask visitors for proof of insurance or a health declaration; most do not. The guides separate what a border actually requires from what is simply sensible.
Activities. Skiing, diving, scooters, altitude and boats each sit behind their own exclusion or add-on.

Europe
Europe is the region where the paperwork question and the coverage question come apart most clearly. Visa-exempt US visitors are generally not asked to show an insurance certificate at the border of Schengen countries, and most European hospitals will treat a visitor first and bill afterward, though public systems still charge non-residents. What decides a bad day is the medical limit, whether mountain rescue or a ferry cancellation is covered, and the rental-car line, because the same trip often crosses several countries. The rest of the world
Outside Europe the range is wider. Some destinations, such as Canada and Japan, have strong hospitals but expect visitors to pay or place a deposit before care. Others, from Caribbean outer islands to Nepal, Patagonia or the Amazon, have limited local treatment, so the real question is how far an evacuation has to go and who approves and pays for it. Weather has a bigger role too: hurricanes, typhoons, cyclones and monsoons drive cancellation and delay claims in ways European trips rarely see. Does travel insurance change depending on the country I visit?
The policy document stays the same, but what it needs to do changes.
